Parent/Child INFORMATION

Welcome! This page will walk you step-by-step through the forms you will need to submit and all of the information that you will need to start the session off on the right foot.  Please note, there are medical forms that need to be completed by your doctor and returned to the school before your child can start.

All forms, including those completed by your doctor, must be received in the office in Bel Air at least 30 days prior to your child's first day in the program.  Children who are returning for additional sessions need only provide the paperwork before the start of every Fall Session.

STUDENT INFORMATION

Please complete and return each of the following required forms by August  1, 2016. Completed forms should be emailed to studentrecords@bmontessori.org or mailed to 2529 Conowingo Rd., Bel Air, MD 21015.

*If you live in a zip code that has been deemed At Risk for lead poisoning, you must return the above Lead Testing Certificate.
